Agency of Accident
This screen is used to add codes and descriptions to identify the cause of accidents/incidents. The Agency (of injury/disease) refers to the object, substance or circumstance directly involved in inflicting the injury or disease. The user can also record on this screen how the injury was sustained and what particular chemical, product, process or equipment was involved in the accident.

- Agency of Accident Details established in this option are used on the Hazards/Incidents and Injuries by Incident screens.
- Agency of Accident Code requires 4 characters or less.

This field displays a 4-character code that represents the cause of the accident.
Description*
This field displays a description detailing the cause of the accident.
